We do have the functionality to create a customized registration form, which can be given a suitable name.

You can create this while creating an assessment as described under How can I create new Assessments? and on the Environment page, you can create a new form. You can add custom fields with different entry types such as radio buttons, drop-downs, text boxes, calendars, etc., or you can simply check the pre-defined fields and set them as mandatory or non-mandatory.
